KY 118, also known as the Tim Couch Pass, begins at an intersection with US 421/KY 80 just to the west of Hyden. This road was built in the vicinity of smaller unpaved roads that ran alongside tributaries of the Middle Fork Kentucky River. The route is mostly hilly and forested, serving as a connector to Hyden.
KY 118's northern terminus is at the Hal Rogers Parkway, one of many parkways in Kentucky. The Hal Rogers Parkway was a toll road until 2003 and toll booths used to line the terminus of KY 118. The 'STOP AHEAD' sign shows its age as the 'PAY TOLL' lettering underneath was removed but still visible at the right angle. Immediately before the Hal Rogers Parkway is KY 3424, a small road that pre-dates the Parkway.
